Allocation of Physician Time in Ambulatory Practice: A Time and Motion Study in 4 Specialties

A direct-observation time and motion study of 57 U.S. ambulatory physicians across family medicine, internal medicine, cardiology, and orthopedics found that during office hours physicians spent only 27.0% of their total time on direct clinical face time with patients and 49.2% on EHR and desk work. For every hour of direct patient care, nearly two additional hours went to EHR and desk work in the clinic day. The paper supports a lightweight, mobile-first schedule surface that lets clinicians glance at their own day without re-entering the desktop scheduler.

Advanced Access Scheduling Outcomes: A Systematic Review

A systematic review of 28 studies covering 24 advanced-access scheduling implementations in primary care found that all 8 studies measuring third-next-available appointment time showed improvements (1.1 to 32 day decreases), and 5 of 11 studies reported significant no-show reductions. Continuity of care and patient satisfaction showed neutral to small positive effects. The paper supports the broader case that visible, well-structured schedule access is a measurable lever for ambulatory primary care performance.
