Screening for Health-Related Social Needs and Collaboration With External Partners Among US Hospitals
It is now widely recognized in medicine that a person’s health is largely determined by social factors, stress exposure, and the environment in which they live, comprising 30% to 55% of health outcomes. The impacts of these social factors have led US hospitals to recognize that effective medical treatment requires attending to patients’ immediate nonmedical needs. These health impacts are commonly divided into 2 levels: health-related social needs (HRSNs) at the individual level and social determinants of health (SDOH) at the societal, community, or population level.
